What Ozempic is

How it works

Semaglutide is a GLP-1 receptor agonist. It mimics a natural gut hormone, GLP-1, that your body releases after eating. That does three useful things: it prompts the pancreas to release insulin when blood sugar is high, it slows how fast your stomach empties, and it signals fullness to the brain. The result is better blood-sugar control and reduced appetite, which is why many people also lose weight on it.

The evidence

Semaglutide was studied in the large SUSTAIN trial program in people with type 2 diabetes. In SUSTAIN-6, published in the New England Journal of Medicine (2016), semaglutide significantly reduced the rate of major cardiovascular events versus placebo, and participants also lost weight compared with placebo. Common side effects across the program were mostly gastrointestinal, nausea, diarrhea, vomiting and constipation, usually worst when starting or increasing the dose.

A 2026 meta-analysis in the International Journal of Obesity found that roughly 20 to 30 percent of the weight lost on GLP-1 medications can come from lean mass. Losing muscle lowers your resting metabolism and makes weight easier to regain, which matters because appetite and weight often rebound after people stop. The fix is well established: resistance training two to four times a week plus adequate protein (research-backed targets are around 1.2 to 2.0 grams per kilogram of body weight daily) preserves muscle during treatment and helps sustain results afterward.
